I think you should do like a anti-radiation syringe, or a medic kit for rad protection is it possible

It would be cool to have a bomb that left permanent radiation!
or make a autofallout map editor that you place early game that makes the top of the map unlivable, kills plants and the like... force a underground dont go to the surface game?

I second that. maybe also spawn a nuclear mutation thing if someone DOES go to the surface (like change them into a zombie like the zombies '09 mod)

What I love of this mod is that this are actual nuclear bombs, with actual fallout and stuff. Now underground bunkers make a lot of sense to me.
It's pretty awesome to create a bunker really deep, and then clear all the life out there with a cobalt bomb.
A small suggestion: Could you create an actor that is radiation-proof? With a gas mask and stuff like that?
That would be awesome.

I dunno if you found out how to check material or whatever, but here's something similar: You could somehow (I'm not 100% sure how) check what kind of wound it has, or rather what that wound emits. Just an idea.
